Download the claim form and submit the completed form to us within 7 days.

Step 3 : Submission of supporting documents

Notify EQI immediately and submit any writ of summons, letter of claim or other legal processes have been issued or commenced against you.

Step 4 : Non-admission of Liability

Do not admit liability or make any settlement with any third party without our prior consent.
