Step 2 : Download Claim Form
Download the claim form within 7 days of the incident and submit the completed form to us within 14 days.

Step 3 : Submission of documents
Submit the photographs of the damaged item(s) together with the original invoice(s) and completed claim form

Step 4 : Luggage Delay/ Damage Claims
For luggage delay/ damage claims, submit to EQI the Property Irregularity Report/ Property Damage and Luggage Tags together with completed claim form.

Step 5 : Submission of documents
Submit the original medical receipt(s) together with completed claim form.

Step 6 : Travel Delay Claims
For travel delay claims, submit to EQI the relevant Airline’s confirmation letter stating the reason and duration of delay together with completed claim form.

Step 7 : Police Report
Submit to EQI a copy of the police report if the loss occurred while not in the custody of the airlines and Transport Authorities together with completed claim form.

Step 8 : Co-operation
Assist and cooperate with EQI’s appointed representative or adjuster.
