Step 1 : Submission of documents

Upon your discharge from the hospital, you are to submit to EQ Insurance Company Limited the following documents:

a) Completed Claims Form

b) Original supporting invoice/receipts

c) Original Final hospitalization Discharge summary

d) Medical Report (if any)

How long will it take for my claims to be reimbursed?

Upon receipt of the completed Claims Form and original supporting documents, EQ Insurance Company Limited will take around 4 weeks to process and finalise reimbursements for all legitimate claims.
