ONLINE PAYMENT USE

1. EQI PAY TERMS AND CONDITIONS

I agree that no reversal is allowed under any circumstances whatsoever, once the payment is charged to my Credit Card.
When I make this payment as a third party settling on behalf at the authorization of the Policyholder, it is understood without dispute any refund due to policy cancellation shall be issued to the Policyholder.
EQI shall not be held responsible or liable in anyway, should there be any dispute arising regarding to such deduction or refund.

2. PERSONAL DATA COLLECTION STATEMENT

I/We agree and consent that EQI may collect, use and process my personal information obtained in the Credit Card Authorization Form and disclose such information to the third party service vendors and financial institutions for the purpose of processing and making payments to EQI.
